The wonderful thing about Rye Brook residents is that they see a need for something in our community and seek to fill that void using their expert knowledge and skills. Enter Jan Zislis, M.D. Dr. Zislis, as many local residents know, is a Doctor at the Greenwich Hospital Emergency Room. What you may not know, is that he recently created Country Doc Concierge. While I had heard of concierge medicine, I didn’t entirely know what it meant, so I sat down with Dr. Zislis to find out what it’s all about and the numerous benefits that come with having a concierge doctor.
Why did you decide to create Country Doc Concierge?
Do you remember the days of old fashioned medicine when healthcare wasn’t so complicated? Well, those days are back! I felt like there was a need for a more personalized approach to healthcare — the way it used to be and should be. I could empathize with the frustration and dissatisfaction many patients felt with our current healthcare system and wanted to change the way health care is delivered. I wanted to provide personalized, quality care in a timely manner and eliminate a lot of the red tape that’s inherent to healthcare today. By keeping my practice size small, I’m able to get to know my patients, their lifestyle, and medical history in a calm and unhurried environment.
What do you mean by “quality care”?
Quality care offered at Country Doc Concierge means a partnership that will go beyond today’s traditional doctor/patient relationship. Patients can contact me via phone, text, or email with their medical questions or concerns.
I can totally relate to the need for that after leaving messages and bouncing from one voice mail to another at my Doctor’s practice.
In addition, there are no time constraints for patient visits and/or sick calls. I want my patients to take a proactive approach to their health. Today so many of us are reactive and call when sick but let’s change that. Let’s change reactive to proactive.

Dr. Zislis is a practicing Emergency Physician on staff at both Greenwich Hospital (Greenwich, CT) and Norwalk Hospital (Norwalk, CT). With his Emergency Medicine background, and his ability to diagnosis and treat both emergent and chronic conditions, his goal is to provide thorough yet practical care to his patients while avoiding unnecessary hospitalizations and ER visits.
Dr. Zislis strives to educate his patients about their medical issues, while also guiding them through complex situations. When patients know and understand their medical condition, it helps to alleviate much of the anxiety associated with the unknown while promoting overall mental and physical well-being.

Can you tell me a little about your background and experience?
I graduated medical school from the prestigious Technion-Israel Institute of Technology in Haifa, Israel, completing a rotating internship in the core specialties of Internal Medicine, General Surgery, and Pediatrics. I then went on to complete my residency training in Emergency Medicine and Traumatology at the University of Connecticut where I received specialized training in both medical and surgical subspecialties. I served as Chief Resident and won numerous awards for my commitment to service excellence.
As a Board Certified Emergency Medicine physician, I am trained in virtually all aspects of health care and possess over 15 years experience in the field. As you know, I am a practicing Emergency Physician on staff at both Greenwich Hospital (Greenwich, CT) and Norwalk Hospital (Norwalk, CT). So what exactly does concierge medicine mean?
A concierge medical practice is one in which a patient pays an annual retainer fee as opposed to a fee-for-service that most of us are all-too familiar with at many doctors’ offices. A personal physician on retainer is accessible 24/7/365 for individuals who desire to have their own doctor who knows them, their medical history, and who can coordinate and facilitate all their care from acute illness to hospitalization.
What services does Country Doc Concierge provide?
- Preventive Care – Doctors can detect a number of medical conditions in their early, treatable stages through screening tests and preventive care.
- Home Care – Most patient services can be done in the home, including blood draws and X-Rays.
- Acute Illness – Sickness or injury may occur at any time during our lives and throughout the year. Whether it is the flu or a minor injury, you are covered.
- Chronic Illness – From diabetes and high blood pressure to migraines and asthma, we can help manage and prevent chronic illnesses for an improved quality of life.
- End of Life Care – Every person deserves to approach the end of their life in a humane and dignified way. I’ll be your advocate providing advice, alternatives and other needs.
- Elder Care Management – Whether you are in need of care, or are caring for an older relative, I will act as a guide and advocate for elder care management.
How do I know if Country Doc Concierge would be right for me?
For the price of a monthly gym membership, our concierge program provides you with an excellent alternative to traditional medical care that is affordable to everyone. I’ll be your advocate and help you navigate the often complex healthcare system so you won’t have to worry. By joining Country Doc Concierge my patients know they can relax knowing they are receiving the care they need by someone who knows your health history and your lifestyle.
So patients pay a retainer fee? What does the retainer fee cover?
The retainer fee varies from each plan, but all patients will be evaluated on an individual basis to determine the best plan for their healthcare needs. For example, a basic plan will cover excellent primary care through:
- A proactive and preventive approach to your health
- Office and sick visits
- Easy telephone, text and email access 24/7/365
- Little or no wait times in the office
- Basic office procedures
- Most in office tests and injections
- Annual wellness exams
- Prompt prescription refills
- Personal referrals to specialists
So what is not covered by the retainer fee?
The following are not covered by retainer fee for concierge medicine services: hospitalizations, evaluations by other physicians or specialists, emergency room visits, prescription medications, outside labs, pathology services, cultures, radiologic studies (X-rays, CT Scans, MRIs, Ultrasounds, etc.), physical therapy and workers compensation. However, these items may be covered by your personal health insurance.
Should I keep my current insurance? Will I be able to use my insurance?
Country Doc Concierge does not participate with any insurance plans nor does the service electronically submit any insurance claims. A paper bill will be provided to all patients for medical care provided. Patients can submit these bills to their insurance company for reimbursement. Please check with your insurance company in regard to reimbursement for out of network providers.
You will still need insurance. Country Doc Concierge is not a replacement for insurance. We highly encourage our patients to have health insurance in addition to our services.
What age group does Country Doc Concierge accept?
Medical services provided by Country Doc Concierge are provided to individuals 15 years of age and older.
Does Country Doc Concierge limit its practice size?
Yes, we do limit the number of patients and it is on a “first come, first served” basis. Once we reach the limit, the practice has a waiting list for those still interested in joining. The enrollment list must be honored in order to provide the highest standard of personalized care and service for those enrolled.
Do you accept Medicare patients?
Yes, Country Doc Concierge will accept Medicare patients; however we don’t submit claims to Medicare for reimbursement.
What if I am hospitalized?
If you are hospitalized, you can choose your hospital. I will work closely with the hospital and consultants to coordinate your care and arrange for close follow-up upon discharge.
Are there extra charges?
Due to insurance regulations prohibiting an all-inclusive plan, you will only receive an additional bill if your services for the year exceed those included in the annual payment. Please note that costs for vaccinations, medications, or other incidentals will be passed on to you.
